Age of Wonders III - really wanted to like it, but the AI was just too simplistic and really dumb.
The one that immediately comes to mind is The Last of Us since I played it recently. It's a great game overall but enemies don't react to your companions AT ALL which can take you right out of the game.
Black and White, though that may have been by design.
I remember how retarded Tales of Destiny group members were, only way I beat the 100 tower was using the acc. that let a friend control another guy in battles.

ANY escort quest AI in the history of games, most of all though fuck you Golden Eye.
And hearthstone ai is pretty bad.
I think the worst part of TLoU, and maybe what supertouch was referring to, is some of the sneaking parts (like the first subway area) you're walking all quiet and sneaking and then Ellie comes up screaming "HEY JOEL WATCH OUT THERES DUDES" and all you can hear is her running like a horse in the subway. None of the monsters react either. Takes the immersion right out. I actually prefer it though, because if they reacted you'd basically have to go through 90% of the game just beating the piss out of everything.
I really hated the companion AI in battles in Ni No Kuni. The options weren't very good on setting them up either. I just used Dino and raped everything after awhile, didn't feel like dealing with them
NWN2, well its camera sucks.
Really all of them. If there is anything I have learned from speedrunning/watching speedruns all games have pretty terrible AI.
I'm going to be honest, this thread should be a white list because I'm pretty sure almost every game should be on this list for shitty AI.
Hl2 had the best ai.

Metro: Last Light was pretty amusing. You pretty much have to stand in front of someone and wave at them before they'll see you. They don't even react to shit like turning off lights, so the old splinter cell trick of "turn off light, strangle guy that comes to turn light back on, another person sees guy, comes to investigate, strangle that guy, repeat until you have a mountain of bodies" doesn't even apply.
Oh, and I managed a great moment like that in The Last of Us where I filled a freezer full of bodies. They just kept walking around that corner. All of them. Like 12+. I wanted to stealth as much as I could in that game just because of the satisfying gurgling noises. It makes me want to go out and strangle a homeless person.

NHL 93 on SNES. Easy to trick the goalie by skating around the goal, effectively making him stuck to one side!
Even better - played the Sega version with a friend and removed all rules basically. Co-op, one of us got the puck and the other one physically mowed away the goalie to give us easypeasy goals.
I just realized that this is AI instead of UI. I blame my phone.
Civilization 5 is top of the list forever. The AI never gets smarter and you can domination victory with 5-6 units as long as you have research parity.
